SPECTRUM OF CARE
Our team understands the experiences of traumatic brain injury survivors. ​The support we provide to you can be brief and simple, in-depth and involved, or something in between.

Step 4: Our team is available for ongoing support
We are here to support you in finding the resources you need to attain access into gainful employment, appropriate community reintegration programs, accessible housing, and research-related education.
EDUCATION
Review our Brain Health Education information or join us for one of our weekly Community Education Sessions.

CASE MANAGEMENT
After completing the above intake form, a team member will contact you to complete registration (which can be completed independently or with assistance).
Following registration, our team will have a better understanding of strengths, weaknesses, and needs to generate a plan for your recovery!
A plan for recovery may include referrals to relevant county services, creation of daily routines, education/information, or incorporation of cognitive strategies.
We can follow up as regularly as you request - from weekly to annually.

ADVOCACY
Your "plan" may include aspects that require support from our team for you to acquire recommended benefits or accommodations at work, at school, or other disability accommodations.
Advocacy may also involve additional evaluations/assessments (e.g., functional capacity, cognitive testing, activities of daily living) to be more effective (if necessary).

We can provide consultations, in-services, or letters of support to your employer, your benefits counselor, your school (for examples).
